TrueMotif apps work by transforming stress into the motivation to engage in actions connected to core, personal values. Brief, avatar-led modules are completed on a mobile phone and followed up by notification messages that “boost” engagement in healthy behavior.

The initial prototype GrowMotif focuses on health behaviors during prenatal and immediate post-natal periods. We chose this as our prototype because targeting healthy behaviors during pregnancy has the largest impact on the health and well-being outcomes of mother and child and greatly reduces future healthcare costs.

GrowMotif is an accessible evidence-based mHealth tool usable on a mobile device that prevents costly birth complications by facilitating care coordination with federal maternal-child health programs. Using the Truemotif engine, GrowMotif has pregnant people complete engaging childbirth prep modules. These modules encourage healthy prenatal behaviors by connecting users to their core personal values and incorporating key public health programs.

GrowMotif delivers value to stakeholders at all levels of the prenatal healthcare system:
* Insurers who provide GrowMotif to their clients will see lower overall healthcare costs, primarily driven by lower rates of admission to the NICU. GrowMotif also adds value by helping Medicaid distributors attract new subscribers in regions with multiple CCOs. GrowMotif helps connect low-SES pregnant people to key public health services, including Medicaid enrollment.
* Healthcare providers benefit from increased and highly efficient patient engagement. GrowMotif helps providers deliver timely, consistent services to meet value-based reimbursement thresholds.
* Pregnant people benefit from a more engaging prenatal care experience. Pregnant mothers want the best for their growing babies and find subscribing to a mobile childbirth preparation program highly appealing. GrowMotif removes barriers to accessing critical pre-and post-natal care by reducing stigma and providing a warm hand-off to key services such as prenatal nutrition services offered through WIC.
Grow Motif improved pregnant mothers’ adherence to the federal WIC nutrition program, which is known to improve infant health outcomes but only when adequately utilized. With Grow Motif, pregnant mothers significantly increased their engagement in the healthy dietary behaviors prescribed by WIC (d = 0.50, p = 0.041). As a result, NICU admissions were nearly 13 times less likely for infants whose mothers received Grow Motif compared to those receiving WIC only (d = 1.41, p = 0.018). Finally, pregnant mothers who received Grow Motif, compared to those who received WIC only, were nearly 6 times less likely to deliver a preterm infant (d = 0.96, p = 0.041) and nearly 12 times less likely to deliver a low birthweight infant (d = 1.37, p = 0.032).
Results of multiple randomized trials conducted since 2013 indicate that perinatal health promotion services boosted with GrowMotif (compared to those without it) reduce NICU admissions and prevent unnecessary c-sections, preterm and low birth weight deliveries, and their associated costs.
GrowMotif works by targeting the primary determinants of poor birth outcomes; prenatal stress and healthy prenatal behaviors, and by increasing engagement in critical prenatal care services. Since our health promotion services are rigorously designed and evaluated for their impact on perinatal social determinants of health. Our research center at the University of Oregon contracts with Medicaid distributors using a value-based payment model that incentivizes healthy patient outcomes that reduce Medicaid distributor costs.
